This study is organized by investigators at Mayo Clinic (Drs. DeLisa Fairweather, PhD and Leslie Cooper, MD- PIs) and in collaboration with Dr. Akira Matsumori, MD (President of the International Society of Cardiomyopathy, Myocarditis and Heart Failure).
The purpose of the study is to identify cardiovascular complications associated with SARS-CoV-2 infection in patients with COVID-19 through an international registry of cases with a special emphasis on clinically suspected myocarditis, cardiomyopathy and heart failure.
Many of the questions can be filled out with a Yes or No. Others will need data input. Please fill out the form with as much detail as you are able to.
